Abstract

Cellular senescence in the aging brain has emerged as a pivotal driver of neurodegenerative disease, linking irreversible cell-cycle arrest and the senescence-associated secretory phenotype (SASP) to synaptic dysfunction, chronic neuroinflammation, and impaired tissue repair. This chapter critically examines the translational landscape for senescence-targeted interventions, including next-generation senolytic small molecules, biologics, and gene-based “suicide” constructs, alongside senomorphic strategies that modulate harmful SASP signaling. We explore the state of biomarker development spanning fluid assays, novel PET/MRI tracers, and peripheral composite scores and detail best practices for preclinical models, early-phase trial design, and regulatory engagement to accelerate clinical translation. Integration with existing neurodegenerative therapies is addressed by discussing preclinical and early human studies that demonstrate synergistic benefits when senescence clearance or SASP attenuation is combined with anti-amyloid, anti-tau, and anti-inflammatory agents while highlighting the importance of intervention timing. Ethical, social, and economic considerations underscore the need for equitable access, long-term safety monitoring, and sustainable healthcare models. Finally, we spotlight emerging technologies such as artificial intelligence, human 3D brain cultures, CRISPR functional screens, and wearable digital platforms that promise to revolutionize biomarker discovery, mechanistic research, and patient monitoring. Together, these insights chart a roadmap for harnessing cellular senescence as a tractable target to preserve cognitive health and enhance the quality of life in an aging population.
